Medication Reviews

  • Comprehensive medication management reviews RMMR for residents of residential care facilities   

  • Provision of Home Medication Review (HMR) 

QUM

  • Audits of medication administration and medication usage. 

  • Drug use Evaluation Audits aimed at assessing and evaluating usage of various therapeutic drug groups. 

  • Information for nursing staff and Doctors on medication administration and medication related issues including, Newsletters, information on new medications, “Do not Crush” lists, laminated reference charts, Treatment room booklets and references. 

  • Education in-services to facility staff on common medical conditions and the use of medications.

  • Access to our online education portal.

  • Involvement in the MAC of the Facility to advise on Policy and Procedures regarding Medication Administration, and legislative requirements. 

  • Accreditation support as requested.

  • Involvement in ensuring that “Quality use of medicine” principles are applied at the facilities.

  • Principles of medication management & best practice guidelines while assisting medications.

ACOP

The aged care on-site pharmacist (ACOP) is funded by the government to provide the service of a credentialed pharmacist who works onsite within a residential aged care facility (RACF).

The credentialed pharmacist will

  • be readily available to aged care staff and residents, building collaboration with the health care team, including general practitioners and community pharmacies.

  • provide continuity in medication management, such as day-to-day monitoring of residents’ medication and solving medication related issues promptly.

  • assist with medication management and communication during transitions of care.

  • undertake whole of facility quality use of medicines activities, such as medicine use evaluations and implementation of changes identified in audits to improve the use of psychotropics, antimicrobials and other high-risk medications.

  • advise, attend & report to the Medicines Advisory Committee (MAC) as part of governance and oversight in the residential aged care home
